Sunset Nursing & Rehabilitation Center Partners with Hospice & Palliative Care to Expand Compassionate End-of-Life Services
Boonville, NY – Sunset Nursing & Rehabilitation Center is pleased to announce a new partnership with Hospice & Palliative Care, enhancing the level of care and support available to residents facing life-limiting illnesses.
This collaboration introduces comprehensive hospice services within Sunset Nursing & Rehabilitation Center, allowing residents to receive comfort-focused, specialized care in a familiar and supportive environment. The partnership reflects a shared commitment to prioritizing dignity, quality of life and compassionate care during life’s most critical moments.
By combining the expertise of Sunset Nursing & Rehabilitation Center’s skilled nursing team with Hospice & Palliative Care’s experienced professionals, residents benefit from coordinated, team-based care that addresses physical, emotional and spiritual needs. In addition to expert pain and symptom management, the partnership provides valuable resources for families, including social work services, spiritual care and ongoing bereavement support for up to 13 months after a loved one’s passing.
This partnership also offers an important option for short-term respite care, affording individuals who care for a patient in their own home the flexibility to travel, recuperate or tend to business. If a patient is currently receiving hospice services, they may be transferred to Sunset Nursing & Rehabilitation Center for up to five days, ensuring they continue to receive compassionate, professional care while their caregiver is away. Hospice respite care is a Medicare and Medicaid-covered benefit for eligible patients who have elected the hospice benefit, subject to applicable coverage guidelines and care planning. Coverage, eligibility, and any applicable cost-sharing are determined based on individual patient circumstances and payer requirements.
