Welcome to Garden Gate Health Care Facility’s Complex Medical Care Unit (CMCU), the first of and only facility in Western New York to offer the Vios monitoring system to improve patient safety and clinical outcomes in a nursing home setting.
Our Goals
As a patient on the CMCU, you will be outfitted with a wireless Vios monitor, which registers real-time cardiac data 24/7, including when you’re at therapy.
The system allows our team of cardiac-trained technicians to monitor essential vital signs, identify changes in your clinical status and provide immediate intervention. This improves your overall outcome and helps avoid a potential readmission to the hospital.
Your Care
On the CMCU, you will be cared for by an interdisciplinary team of cardiac specialists including doctors, registered nurses, therapists, technicians and
certified nursing assistants. This team of specialists is committed to individualizing your care with a focus on safety, healing and rehabilitation.

Roadmap to Discharge
From the moment you arrive, our goal is to guide you on a personalized path toward recovery and a safe return home. Our interdisciplinary team — including doctors, nurses, therapists, dietitians, and support staff — works together to design a care plan tailored to your needs. Every step of the way, we focus on maximizing your independence, building strength and confidence, and ensuring a smooth transition back into the community.
- Reviews medications, care needs and communicates concerns to doctor
- Explains and provides any scheduled treatments
- Facilitates follow-up appointments and transportation needs
- Completes evaluation and reviews activities of daily living
- Determines goals with patient, along with home needs

- Creates personalized plans after detailed evaluation
- Implements programs to enhance speech, memory, and swallow function
- Facilitates referrals for further swallow diagnostic testing as needed

- Explains process of family meeting with interdisciplinary team and/or screening procedure
- Assesses all food preferences and nutritional needs

Discharge from the facility is typically available Monday through Thursday between the hours of 8AM-4PM. In order to ensure a safe discharge, Friday and weekend discharges are typically not available due to accessibility of homecare and equipment services on weekends. Our nursing and therapy teams will schedule any family training that may be needed before your discharge date. We will also order all medications and assist in coordinating follow-up appointments with your primary care physician and other providers.
If you’re considering elective surgery, pre-planning your stay will allow you to seamlessly transition from the hospital to our facility.
