We’re pleased to offer cardiac rehabilitation for individuals with heart disease, congestive heart failure and for those who are leaving the hospital after surgery or a heart attack. Our program is patient-focused, offering you a comprehensive, customized program for healing, exercise and education.
Our cardiac rehabilitation program is interdisciplinary, meaning we utilize a team of health care providers such as a physiatrist, physical therapist, counselor, nurses, dietitian and other medical staff to develop an individualized plan of care focused on maximizing your recovery and setting attainable goals.
One of the most important aspects of cardiac rehabilitation is setting long-term goals and exercise plans for healthy living. Our team will work with you to develop a realistic plan for eating healthy, controlling weight, lowering your blood pressure and cholesterol levels, eliminating bad habits, and managing stress. This will not only help increase your physical fitness and improve your overall health, but also may reduce the risk of future heart issues.

In addition to the post-acute cardiac care offered at our facilities, Garden Gate Health Care Facility in Cheektowaga proudly offers a Complex Medical Care Unit (CMCU) designed for individuals recovering from heart-related illnesses, surgeries, or deconditioning due to cardiac distress. The CMCU is equipped with the Vios Monitoring System, an advanced wireless technology that continuously tracks real-time cardiac data 24 hours a day, even while residents are in therapy.
Each patient is fitted with a wireless Vios monitor that records vital signs such as ECG, heart rate, blood oxygen level, pulse rate, respiratory rate, and posture. This data is monitored by cardiac-trained technicians, allowing the care team to detect changes instantly and respond immediately. The result is improved safety, reduced risk of complications, and fewer hospital readmissions.
Patients on the CMCU are supported by an interdisciplinary team of Doctors, Registered Nurses, Therapists, Technicians, and Certified Nursing Assistants. Together, they create personalized care plans focused on healing, rehabilitation, and long-term heart health.

Roadmap to Discharge
From the moment you arrive, our goal is to guide you on a personalized path toward recovery and a safe return home. Our interdisciplinary team — including doctors, nurses, therapists, dietitians, and support staff — works together to design a care plan tailored to your needs. Every step of the way, we focus on maximizing your independence, building strength and confidence, and ensuring a smooth transition back into the community.

Discharge from the facility is typically available Monday through Thursday between the hours of 8AM-4PM. In order to ensure a safe discharge, Friday and weekend discharges are typically not available due to accessibility of homecare and equipment services on weekends. Our nursing and therapy teams will schedule any family training that may be needed before your discharge date. We will also order all medications and assist in coordinating follow-up appointments with your primary care physician and other providers.
If you’re considering elective surgery, pre-planning your stay will allow you to seamlessly transition from the hospital to our facility.
